What You’ll Do
The NDIS Support Coordinator plays a vital role in assisting participants to implement their NDIS plans by connecting them with appropriate supports and services. This position supports a diverse range of participants—from those requiring general coordination and capacity building (Level 2) to those with complex needs needing specialist support coordination (Level 3). The Support Coordinator empowers participants to exercise choice and control, manages service barriers, and ensures continuity of care aligned with NDIS standards.
-Assist participants to understand and implement their NDIS plans, including budgeting and goal planning.
-Coordinate and connect participants with informal, mainstream, and funded supports and service providers.
-Support participants in developing service agreements and managing bookings.
-Provide hands-on assistance for participants to build skills and independence in managing their supports.
-Manage complex cases involving participants with high needs, multiple service systems (e.g., health, justice, housing), or crisis situations.
-Develop and implement risk mitigation and crisis response plans where necessary.
-Collaborate with allied health professionals, families, and external agencies to ensure holistic and coordinated care.
-Advocate for participants’ rights and support needs within complex service environments.
-Monitor and evaluate service effectiveness, adjusting coordination strategies as needed.
-Maintain detailed, NDIS-compliant documentation and reporting.

-Experience and Knowledge
-Proven experience in support coordination, disability services, or community support roles, with solid understanding of NDIS frameworks and person-centered practices.
